<p style="text-align: left;">Growing older is a beautiful and natural part of our life process. Our older years can be very satisfying, but they may also bring challenges and concerns, including higher levels of family stress, deterioration in relationships, inadequate use of health services and premature institutionalization. When the care of an older relative has reached a point of being too complex to manage, a geriatric care management program provides peace-of-mind.</p>
<p>Through the <strong>Family Connections Program</strong>, the Jewish Family Services Care Management team assists older adults and their families as they plan the care needed to maintain the highest level of independence, safety, dignity and quality of life.</p>
<p>The <strong>Family Connections Program</strong> is a fee-for-service program designed to provide support, education, guidance, and service coordination to dispersed and local family members and ensure that proper support services are in place to help keep their older adult relatives as independent as possible.</p>
<p>The Family Connections Care Management team will help define the needs of your loved one and find support services in the home and in the community, that address the physical, mental and emotional challenges of aging.</p>
<p>The Family Connections program includes several components as follows:</p>
<ul>
<li>Family Consultation and Geriatric Assessment &#8211; The Clinical Therapist will conduct a comprehensive in-home evaluation to assess your loved one’s cognitive, physical and emotional status.</li>
<li>Personalized Plan of Care &#8211; The Care Manager will develop a personalized plan of care, including recommendations of services, referrals and resources.</li>
<li>Ongoing Care Management &#8211; After presentation of the plan of care, you may choose to select Jewish Family Services to provide ongoing care management services. Our Care Managers will use their expertise in elder care and navigating systems to implement the plan of care and adjust to your loved one’s changing needs.</li>
</ul>

<p>Family Connections is the only geriatric care management program in the Charlotte Jewish Community. It is funded in part with a grant from BJH Foundation for Senior Services and the Jerome Madans Fund and the Stan Greenspon Charitable Fund.</p>
